New Patients

Please fill out the intake form and return it to me ideally before attending our first meeting. This is the only required form. Here is a release of information if you would like me to speak with anyone (e.g. doctor, school personnel, etc…) regarding our work. Here is a HIPPA document that discusses your rights and privacy regarding your information. Here is a document that outlines counseling and specifically addresses late fees, how to contact me during an emergency, and confidentiality. I am happy to answer any questions you might have regarding the linked forms.
